Who We Are!

Sue McCann is a masters prepared board-certified Family Nurse Practitioner with over 15 years of experience caring for older adults. With a background in emergency medicine, hospital medicine, palliative care and geriatrics she is an expert in helping families navigate the challenges that accompany a parent’s changing health needs. Dementia Navigators LLC was founded by Sue McCann, a Nurse Practitioner and daughter of a mom with Alzheimer’s disease.

The goal of Dementia Navigators is to remove some of the worry and stress that comes when a parent develops medical conditions that interfere with their ability to care for themselves. Using her knowledge of the healthcare system, medical background, advanced planning skills and experience helping families navigate difficult decisions Sue helps families navigate the transitions that accompany a parent with changing health needs.


Who is this service for?

Your parent does not have to have dementia in order for you to benefit from Dementia Navigators expertise. The transitions package is for families struggling with challenges related to aging parents such as change in health, caregiver burnout, increased care needs, transitioning out of the home or advanced planning. The driving dilemma is for families whose parents may be having difficulty driving safely due to medical conditions. The medical condition can be related to either memory loss or physical challenges.
